Parent Consent for School Health Services Form and Emergency Care Cards are needed to receive your child's teacher's name. You can pick these up in the office.
NWES 2023-2024 Teacher Request Forms 👩🏫
Please use the link for the grade level your child will be in during the 2023-2024 school year. Parents are encouraged to take advantage of the opportunity to complete a request form, even if they think their child will qualify for REACH. REACH placement fluctuates throughout the summer as new students enroll and other students withdraw. Timeliness and completeness of request forms do impact placement, so be sure to complete your request by June 18th, and select the number of teachers required.
